HOW TO ACCESS OUR SERVICES

PLEASE READ THIS ENTIRE PAGE – As an applicant you are responsible for knowing this information.

For emergencies – life-threatening conditions that require immediate treatment, such as severe injuries and acute illness, call 360-774-2152.

For all other conditions such as mild illness or infection, skin issues, spay, neuter, microchip, licensing, dental care, flea treatment, ear mite treatment, mange treatment, etc., please email your answers to the following questions to pethelperspt@gmail.com

PLEASE ANSWER THE FOLLOWING QUESTIONS IN FULL DETAIL:

  1. What is your full name, phone number, email address, and address of residence?
  2. How many pets do you have? Please list each pet individually with name, age, gender, breed, approximate weight, color.
  3. What is your pet’s issue for which you are asking assistance?
  4. Have all your pets been vaccinated?
  5. Are they neutered or spayed? __yes   __no but willing to have it done.
  6. Do you have a carrier (cats and small dogs) or leash (larger dogs)?
  7. Will you need transportation to your vet appointment?
  8. Do you agree not to acquire any new pets while the ones you currently have are still living?
  9. Do you agree to call, text or email the main office for referral (not the vet) every time you need a new appointment?
  10. Are you employed? On Social Security? Disability? Other?
  11. How many people are living in your household?
  12. What is the total monthly income of all household members including yourself, and from all sources (including employment, disability income, Social Security, food stamps, support from family living elsewhere, and housing allowances)?
  13. Do all household members share that income?
  14. Do you support anyone not in your household?
  15. What is your housing situation  –  do you rent, own, in subsidized housing, transitional housing, live with relatives or friends, no home?
  16. What are your household’s  monthly expenses for: rent_____ internet_____ phone_____ health insurance______  car insurance______ ongoing medical bills_____ utilities (electricity, trash, water)_______
  17. Can you pay part of the veterinary cost? How much? _____Do you agree to pay that amount to the veterinarian at the time of the appointment?

If you qualify for our assistance, we will refer you to a specific veterinarian so you may make an appointment. Pet Helpers may pay at least part of the cost, directly to that veterinarian only. We do not pay retroactively for veterinary visits already done, or for unapproved veterinary visits.

We will assess your answers to the above questions, and may help you if you qualify and:

You live in Jefferson County, Washington State

You contact us BEFORE taking your pet to a veterinarian so we can direct you to the vets who work with us

You cannot afford to pay for veterinary costs (we may pay all or part)

You are willing to comply with all veterinary clinic policies

Your pet is not a danger to other animals or people

You are willing to have your pet in a carrier or on a leash for the vet appointment

You agree to not acquire additional pets while you have the one(s) Pet Helpers had agreed to help with.

If you need it, transportation to the veterinary clinic may be available for you and/or your pet
